What is TMJ?

TMJ stands for the Temporomandibular Joint.

The temporomandibular joint (TMJ) is a hinge type synovial joint that connects the mandible, or jaw to the skull. It is a special type of synovial joint, as it is lined Fibrocartilage different from most other joints. It also contains an articular disc that helps with cushioning of the joint with movement and loading. The way this joint and surrounding muscles are designed and move allows us to bite, chew, eat, speak, express and interact. It may affect your sleep. If you take away this function or limit it, this could greatly impact your physical, social and emotional health and well-being.

What can go wrong?

When you have any trouble or pain in this joint it is referred to as TMD.

TMD stands for temporal-mandibular disorder.

Signs and symptoms of TMJ disorders may include:

  • Pain or tenderness of your jaw
  • Pain in one or both of the temporomandibular joints
  • Experience pain or sensation in the face, cheek, teeth, ear or head/headache.
    • This may include a fullness or tightness feeling in the face, sinuses or ear;
    • Ringing, burning, aching
  • Clicking, snapping or grinding/grating
  • Have difficulty or pain while chewing
  • Limiting your ability to open or close your mouth

Causes and Risk Factors

Painful TMJ disorders can occur if:

  • There is wear and tear on the disk or joints cartilage
    • Arthritis
    • The joint is damaged by injury from impact or extreme and strenuous bite or load
  • The disc gets pulled out of its normal position

Risk factors

Factors that increase the risk:

  • Various types of arthritis
  • Jaw injury
  • Long-term grinding, clenching of teeth or any other oral habit that may not be consciously done like sucking or biting the cheek or tongue.
  • Chewing gum, ice or other objects
  • Certain connective tissue diseases that cause problems that may affect the temporomandibular joint
  • Malalignment of the teeth and joint
  • Poor posture in the neck and spine

Typical assessment includes:

  • Joint assessment, mobility and irritability of the jaw, c-spine
  •  Other postural factors in the neck, shoulder mid and low back
  •  Soft tissue and trigger points palpation intra and extra-orally
  •  Range of motion of the jaw and cervical spine
  •  Deep neck flexor function and endurance

Treatment Options With Physical Therapy

  • Soft tissue and trigger point release of the masticatory muscles, neck and scapular/shoulder region
  •  Trigger Point Dry Needling
  •  TMJ and cervical joint mobilization
  •  Ultrasound
  •  Iontophoresis

    • Addressing contributing issues within scope of practice, e.g. anxiety and diet; deep breathing and nervous system calming

    • This could include but not limited to splinting or an appliance device, or injections. In more extreme cases discussion of orthodontics to correct alignment may be necessary.

How long does TMJ and jaw pain last?

The duration of TMJ pain varies based on individual cases and their underlying causes. Left untreated, TMJ pain can persist throughout one’s life, leading to increased strain on the joint and surrounding tissues. Typically, we see patients for 4-8 weeks. During this period, you will get pain relief and regain functions like eating, talking, sleeping, and chewing.

Additionally we can collaborate with dental specialists to assess and fit patients with a customized appliance or splinting device for additional relief, if needed. We equip patients with essential knowledge and tools to manage any recurring symptoms effectively. Our comprehensive approach ensures not only short-term relief but also long-term strategies for sustained TMJ management. Contact us today to begin your journey towards a pain-free, functional life.
